Few topics in men’s health spark as much confusion or controversy as hormone replacement therapy (HRT). Testosterone is the primary male sex hormone, but its influence extends far beyond libido and muscle mass. It shapes energy, mood, motivation, bone density, red blood cell production, and even metabolic health.
When levels decline, men don’t just feel tired or less interested in sex, they often lose the physiological drive that keeps them strong, lean, and engaged with life.
Despite that, men’s HRT remains one of the most misunderstood areas of medicine. For decades, doctors avoided prescribing testosterone because of fears it might increase the risk of prostate cancer or heart disease.
Meanwhile, the pendulum has swung the other way online, where influencers and “biohackers” promote testosterone replacement as a universal solution for every man over 30. Both views are wrong, and both can cause harm.
The truth is more nuanced. Properly prescribed and monitored, testosterone therapy can be life-changing for men with genuine deficiency. It can restore vitality, preserve muscle and bone, and protect against metabolic decline. But it’s not a shortcut, and it’s not without trade-offs. Understanding when and how to use it responsibly is critical.
The Myth of Inevitable Decline: Why Testosterone Doesn’t Have to Fall with Age
If you ask most men, they’ll tell you testosterone naturally plummets as you get older—that it’s just part of aging. But research shows that’s not entirely true. In healthy men, testosterone levels rise sharply during adolescence, peak in the late teens to early twenties, and then remain remarkably stable through midlife and beyond.
A large model drawn from thirteen studies (illustrated below) shows that total testosterone peaks at about 15.4 nmol/L (≈445 ng/dL) around age 19, and by age 40, averages only modestly lower at 13.0 nmol/L (≈375 ng/dL). From there, levels stay relatively flat through old age, with variation rather than steady decline being the more striking trend.
In other words, healthy aging doesn’t automatically mean low testosterone; what changes is how consistently men maintain it.
That variation reflects the growing divide between men who stay metabolically healthy and those who don’t. Lifestyle and environment—not age alone—are the main culprits behind falling testosterone in modern men. Factors like excess body fat, insulin resistance, poor sleep, alcohol, stress, and endocrine-disrupting chemicals can all suppress testosterone production and increase SHBG (sex hormone–binding globulin), which ties up free testosterone and makes less of it available to tissues.
Supporting data from the 2011–2016 National Health and Nutrition Examination Survey (NHANES) confirm this. In a representative sample of U.S. men aged 20–44, median testosterone levels declined slightly by decade—from roughly 558 ng/dL in men in their 20s to around 473 ng/dL by their early 40s—but there was massive overlap across age groups. Many 40-year-olds had higher T than men 15 years younger.
The real takeaway is that age-specific ranges matter, but lifestyle matters more.
Finally, not all men respond to testosterone the same way. Differences in androgen receptor sensitivity due to genetic variations mean that one man may thrive at 400 ng/dL while another struggles at 600. That’s why numbers alone can’t tell the full story. Symptoms, function, and quality of life must guide the interpretation.
In short: testosterone doesn’t have to decline dramatically with age. What determines the trajectory is how you live, train, sleep, and eat.
The Safety Myths of Testosterone Therapy
Let’s start with the heart. Early observational studies once suggested that higher testosterone might raise cardiovascular risk, but these studies were small, uncontrolled, and often confounded by the fact that men receiving testosterone already had more health problems to begin with.
Modern data paint a very different picture. A meta-analysis of 30 randomized controlled trials involving over 11,500 men found no increase in cardiovascular disease, heart attack, stroke, or mortality in men using medically supervised TRT compared with placebo.
In fact, men with low testosterone often have worse cardiovascular outcomes. Low-T states are linked with visceral fat accumulation, insulin resistance, inflammation, and endothelial dysfunction, which are all precursors to heart disease. NHANES data from over 10,000 men shows that having low testosterone levels greatly increases the risk of cardiovascular-related death compared with those in normal ranges.
The key distinction is dosage and intent. Clinical TRT replaces hormones to restore physiologic levels, while anabolic steroid abuse pushes testosterone into supraphysiologic ranges. In contrast to the safety of TRT, a large population study of nearly 60,000 men found that anabolic steroid users had dramatically higher risks for cardiovascular events—including heart attack (3× higher), arrhythmias (2×), cardiomyopathy (9×), and heart failure (3.6×).
The message is simple: restoring testosterone is safe; abusing it is not.
The second fear—that testosterone fuels prostate cancer—dates back to flawed logic from the 1940s, when physicians observed that removing testosterone (through castration or hormone suppression) slowed advanced prostate cancer. For decades, that observation was misapplied to all men, regardless of health status.
Modern research has overturned that belief. A major population-based analysis using SEER-Medicare data followed over 6,800 men with prostate cancer who were managed on active surveillance. Of these, 167 received testosterone therapy. After a median follow-up of five years, there were no prostate cancer–specific deaths in the testosterone group versus 0.6% in the untreated group. TRT was actually associated with a 34% lower likelihood of progressing to active treatment and showed no increase in overall mortality.
So, for men with stable or low-grade prostate cancer under medical supervision, physiologic testosterone replacement does not increase cancer risk and may even improve outcomes when appropriately monitored.
Third, testosterone can increase red blood cell mass, raising hemoglobin and hematocrit. That’s not inherently dangerous but can thicken the blood if levels climb too high. Regular monitoring prevents this. For most men, simple adjustments like switching from large weekly injections to smaller, more frequent doses keep hematocrit in range. Others may donate blood occasionally if needed. With appropriate follow-up, this is a manageable, minor consideration rather than a safety issue.
The real danger isn’t testosterone therapy, it’s leaving low testosterone unaddressed. A large systematic review of over 350,000 participants found that higher testosterone levels were associated with an 11% reduced risk of death from all causes.
In other research involving over 10,000 U.S. men, having higher testosterone levels was linked to lower risks of dying from all causes, cardiovascular diseases, cancer, influenza, Alzheimer’s disease, and cerebrovascular diseases.
In other words: when testosterone levels are restored to physiologic ranges under medical supervision, men don’t just feel better, they live better and longer.
The Estrogen Question and Why Aromatase Inhibitors Can Backfire
When most men think of estrogen, they think of it as a “female hormone.” But that’s outdated thinking. Men need estrogen too, just not in the same way women do. The predominant form, estradiol (E2), is produced when testosterone is converted by the enzyme aromatase, and it plays a crucial role in male physiology.
Estradiol is essential for libido, erectile function, brain health, and even sperm production. Estrogen receptors and aromatase enzymes are found throughout the brain, penis, and testes, which are the organs that collectively drive sexual function and reproduction. In the brain, estradiol helps modulate arousal and mood; in the penis, receptors are concentrated around the neurovascular bundles, helping regulate erection quality and blood flow; and in the testes, estradiol fine-tunes sperm development through a delicate balance of stimulation and inhibition across the hypothalamic–pituitary–gonadal axis.
Like most hormones, balance is everything. Too little estrogen and libido plummets; too much, and symptoms like water retention or gynecomastia may appear. But what’s often missed is that both extremes are harmful for sexual health.
Data from NHANES (954 men followed for 25 years) showed that lower estradiol was associated with higher cardiovascular mortality. Each 1 standard deviation drop in estradiol increased the risk of CVD death by 2.3-fold. Similarly, men with low free estradiol had nearly double the cardiovascular risk.
On the other hand, a study of 547 men seeking evaluation for new-onset erectile dysfunction found that roughly 18% had hyperestrogenism—estradiol levels above 42.6 pg/mL. These men had more severe erectile dysfunction, higher comorbidity scores, and higher rates of hypogonadism. Estradiol was positively correlated with total testosterone but negatively correlated with erectile and orgasmic function, meaning that men with excessively high estrogen—even with decent testosterone—had worse sexual outcomes.
Yet the solution isn’t to “nuke” estrogen with aromatase inhibitors (AIs). Over-suppression can be equally damaging. A large clinical review from a sexual medicine practice found that only 2.6% of men on testosterone therapy required an AI, typically for estradiol levels above 60 pg/mL or between 40–60 pg/mL with symptoms. When treated with low-dose anastrozole (0.5 mg three times per week), estradiol dropped from a median of 65 pg/mL to 22 pg/mL, while testosterone levels remained stable.
All this is to say that elevated estradiol is rare in properly dosed TRT, and when it occurs, it’s manageable without blanket suppression.
Gabrielle’s practical approach is simple:
- Don’t start an aromatase inhibitor “just because you’re on TRT.”
- Aim for estradiol levels around 30–50 pg/mL.
- Use an AI only when estradiol remains persistently high and symptoms suggest a true problem, usually mild gynecomastia or water retention.
It’s also worth noting that obesity is one of the most common drivers of elevated estrogen in men. Fat tissue expresses aromatase, meaning the more adipose tissue you carry, the more testosterone you convert into estradiol. That doesn’t mean the answer is to block estrogen; it means treating the root cause, which is the low testosterone and metabolic dysfunction driving fat gain in the first place.
Fertility and Testosterone Therapy: What Every Man Should Know
One of the biggest misconceptions about testosterone replacement therapy (TRT) is that it makes men infertile forever. That’s not true, but it’s not something to take lightly either.
Testosterone production is governed by a tightly regulated feedback loop between the brain and the testes. The hypothalamus releases GnRH, which signals the pituitary to secrete LH (luteinizing hormone) and FSH (follicle-stimulating hormone). These two hormones tell the testes to make testosterone and sperm.
When you start taking testosterone from an external source, the brain senses that levels are sufficient and shuts down LH and FSH production. Without those pituitary signals, the testes go quiet: natural testosterone and sperm production stop, and the testicles often shrink slightly from disuse.
The good news is that these effects are reversible for most men, especially when fertility preservation is planned in advance.
An integrated analysis of 30 clinical trials found that men who discontinued TRT recovered sperm production at predictable rates:
- 67% regained fertility (>20 million sperm/mL) within 6 months
- 90% within 12 months
- 96% within 16 months
- 100% within 24 months
So, fertility recovery is the rule, not the exception, but it can take time.
If you want to recover even quicker, a clinical trial of infertile men who had been on testosterone therapy for an average of 4.5 years found that administering 3,000 IU of human chorionic gonadotropin (hCG) every other day restored fertility in about 4.5 months on average.
hCG is a hormone almost identical in structure to LH, the chemical messenger your brain normally sends to your testes. Administering hCG “stands in” for LH, keeping the testes active even when the brain has stopped sending its own signal. In other words, hCG keeps the machinery running while TRT takes over the hormone output.
Research shows that 250–500 IU of hCG per day is enough to maintain normal intratesticular testosterone levels, even when natural LH and FSH are completely suppressed. For men who prefer a less frequent regimen, the American Association of Clinical Endocrinologists recommends 2,000 IU of hCG three times weekly as an alternative to TRT in men with hypogonadism who want to preserve fertility.
Combining hCG with TRT has been shown to prevent sperm count decline entirely. In one study, men receiving 500 IU of hCG every other day alongside testosterone therapy maintained normal sperm concentrations for at least four months, even as their total testosterone exceeded 1,000 ng/dL.
Another study in healthy young men compared testosterone enanthate (200 mg/week) with varying hCG doses. Those receiving 250 or 500 IU of hCG every other day maintained normal intratesticular testosterone levels, while those on testosterone alone saw them collapse.
For men who want to boost testosterone without suppressing fertility, clomiphene citrate (Clomid) is a proven alternative. It works by stimulating the pituitary to release more LH and FSH, naturally increasing testosterone production.
In a prospective trial of 125 men with low testosterone and low libido, daily 25 mg Clomid raised serum testosterone from 309 ng/dL to 642 ng/dL in just three months, with improved sexual function and no serious side effects.
A broader meta-analysis of 19 studies (over 1,200 men) found that clomiphene increased total testosterone 2.6-fold while also raising LH, FSH, and estradiol. Improvements in symptoms and quality of life were consistent, and adverse effects were rare (<10%).
Long-term data are also reassuring. In a study of 400 men treated for up to seven years, 88% achieved normal testosterone levels, 77% reported symptom improvement, and only minor side effects (mild mood changes or breast tenderness) occurred in a handful of cases.
Finally, when directly compared with TRT, clomiphene raised testosterone into the normal range (to around 500 ng/dL) and produced similar satisfaction scores to injections or gels, despite achieving slightly lower peak levels. Libido improvements were greatest with injections but overall well-being was equivalent across therapies.
If fertility is important to you—now or in the future—talk to your doctor before starting TRT.
- If you plan to have children soon: Bank sperm or consider hCG or Clomid monotherapy instead of TRT.
- If you’re already on TRT: Adding 250–500 IU of hCG every other day (or 2,000 IU 2–3×/week) can keep your testes producing sperm. You may also consider 25 mg of clomid daily to increase your own LH and FSH levels.
- If you come off TRT: Expect 3–7 months for fertility to rebound naturally, though recovery can be accelerated with hCG.
Testosterone therapy and fertility don’t have to be mutually exclusive. With smart planning and medical supervision, men can maintain hormonal health and reproductive potential at the same time.

Hormones operate in a tightly interlinked network, and what looks like “low testosterone” on paper can sometimes be a symptom of something else entirely. Start with a morning, fasting blood draw. Testosterone peaks early in the day and drops as the hours go by, and even something as simple as eating breakfast can reduce measured levels by 10–20%.

- Gels and patches: Simple to apply, offering steady release through the skin. The major drawback is the risk of transference. Partners or kids can absorb it through skin contact (and no one wants their toddler growing a beard). Many men who start with gels eventually switch to injections for better control and predictability.
One of the most important parts of starting therapy is managing expectations. Testosterone replacement isn’t a switch you flip, it’s a slow recalibration of your biology.
Most men notice subtle changes within two to four weeks: better sleep, steadier mood, increased drive, and improved workouts. By three months, energy, libido, and body composition changes become more noticeable. The key is patience and consistency. Blood levels and symptoms are reassessed regularly to fine-tune the dose until you find your “steady state.”
The goal of HRT isn’t chasing the highest possible number, it’s restoring the balance that allows your body and brain to function optimally. With the right testing, the right delivery method, and proper follow-up, testosterone therapy becomes not just effective, but sustainable.
